Personal Care
Delivered in a person-centred approach, meaning the needs, preferences, and goals of the individual are at the forefront of care planning and delivery. This involves creating a personalised care plan for each individual, which details their specific needs and how these will be met.
Trained care workers deliver the care as part of a multi-disciplinary team that includes nurses, social workers, and therapists. They work collaboratively to ensure the individual's physical, mental, and emotional well-being is catered to.

The delivery of personal care also involves regular reviews and updates to the care plan, taking into account any changes in the individual's condition or circumstances. This ensures that the care provided continues to meet the individual's needs over time.
In the case of Living Waters, we use a digital system to update care tasks and medication changes immediately and to access care plans. This enables them to deliver person-centred care effectively and in a timely manner.
Our Approach
Collaboration is essential in delivery of our TDDI service. This involves working collaboratively with a range of professionals such as GPs, Psychiatrists, Occupational Therapists, and the Mental Health team to provide consistent, effective, timely care.
This collaborative approach helps to ensure that service users have access to the full range of clinical and mental healthcare services they need.

Training and Competency Assessments:
To ensure that staff are capable of supporting clients with complex care needs, Living Waters provides both mandatory and specialist training. This includes training in specific care techniques such as enteral/PEG feeding, stoma care, gastrostomy feeding, nasopharyngeal suctioning, oral suctioning, bowel management, oxygen therapy, and tracheostomy care.
Staff competency is regularly assessed, and their day-to-day support of clients is monitored.

Collaborative Care: Modern Facilities & Personalised Support.

Initial Assessment.  
At the outset of our onboarding process, we meticulously review the referrals to pinpoint the exact needs of the child or young person and evaluate how best we can extend our support. This stage is crucial to ensure that our services are in alignment with the requirements of the young individual.
Professional Collaboration & Transition Prep.
Upon identifying a potential match, we convene with professionals, discuss the child's needs, and initiate a transitional process. This includes sharing a team booklet with the child and communicating vital information with professionals to develop support and care plans, while also coordinating with external agencies.
Placement & Personalised Support Plan
After the child's placement, we perform a mental health assessment in our initial evaluation phase. This critical step establishes the foundation for personalised care by ensuring the provision of support tailored to the young person's distinct needs and challenges.
