Upon identifying a potential match, we convene with professionals, discuss the child's needs, and initiate a transitional process. This includes sharing a team booklet with the child and communicating vital information with professionals to develop support and care plans, while also coordinating with external agencies.

After the child's placement, we perform a mental health assessment in our initial evaluation phase. This critical step establishes the foundation for personalised care by ensuring the provision of support tailored to the young person's distinct needs and challenges.
